Position Description

This is a full time, exempt social work position. The qualified candidate will possess a balance of clinical, communication, organizational and administrative experience.  This position is responsible for providing social services component, oversight of community outreach functions and serves as a member of the management and multidisciplinary team under the direction of the Program Director.    

Clinical Support:  This position shall provide information, resources and supportive services to potential and active participants and their families that help facilitate successful enrollment and participation in the program or provide referrals for other needed services.  Duties include:

Supervision of social services personnel and interns.

Serve as a member of the management and multidisciplinary team.

Interview and screen all applicants/referrals to determine their appropriateness for the full assessment process and participation in the adult day health care or social day program.

Provide referrals as needed for potential, active and ineligible families.

Conduct an initial home evaluation, assessment and subsequent reassessments of each participant.

Prepare an individual plan of care for each participant, if social services are to be provided.

Maintain the social services component of the participant health records as required.

Provide counseling to participants and lead participants in problem oriented discussion groups.

Conduct a caregiver support group twice monthly.

Serve as a liaison with participants' families/significant others and/or care providers.

Serve as a liaison with other community agencies to assist families.  

Serve as a member of the Utilization Review, CQI and Scholarship Assistance Committees.

Perform other functions as indicated by licensing regulations and IADHS policies.

Perform other duties as required by the program or executive director.

Community Outreach:  This position shall develop and implement community outreach activities that promote IADHS as a qualified health care option for older and disabled adults and caregivers and as a valued community partner. 

·Advise and make recommendations on strategic community outreach and marketing activities.

·Maintain relationships through outreach visits with referral resources on a regular basis.

·Develop and maintain a referral resource file and report monthly on community outreach contacts.

·Develop and maintain collateral material; Implement and manage the collateral distribution plan.

·Conduct facility tours and arrange and facilitate community education workshops.

Qualifications

·         A bachelor's or master's degree in social work with three years experience working in one or more fields of aging, health, or long-term care. 

·         Knowledge of the physical, mental and social needs of the elderly and other functionally impaired adults and has demonstrated competence in working with them.

·         Possess strong organizational skills and be proficient in Word, Excel and other productivity software.

·         Must be able to speak, read and write English fluently. Bilingual skills are strongly desired.

·         Able to properly lift 25 pounds.
